Grip strength and appendicular lean mass appear repeatedly as independent predictors of mortality after adjustment for age, BMI, and activity level. Observational data cannot prove that adding muscle extends life, but the consistency across cohorts is unusual.

The intervention data is more modest and more actionable: two resistance sessions per week, taken near the limit of what can be completed with control, produce meaningful strength and lean-mass gains in previously untrained adults across every age group studied, including the ninth decade.

During weight loss — particularly on GLP-1 receptor agonists — resistance training and adequate protein are the two levers with evidence for preserving lean mass. A large share of unmanaged rapid weight loss is not fat.
