The Mediterranean Pattern Has the Best Trial Support

It is the only whole dietary pattern with a large randomised trial reporting cardiovascular endpoints.

The Mediterranean pattern remains the only whole diet tested for hard cardiovascular endpoints at scale.

Most diet research is observational because randomising thousands of people to a whole diet for years is expensive and hard to blind. One large trial did it, with olive oil or nuts added to a Mediterranean pattern.

It reported fewer cardiovascular events in the intervention arms. It was retracted and republished after randomisation irregularities at some sites, with the main conclusion surviving reanalysis.

The pattern is unremarkable in composition: vegetables, legumes, whole grains, fish, olive oil as the main fat, limited red and processed meat, and moderate dairy.

That it has the strongest support says as much about how little else has been tested as it does about the diet.
